United Methodist Homes has an opportunity for a Part-Time LPN Facility Charge Nurse (24 hours per week).  This position is on evenings and works every other weekend.

This position performs and coordinates a variety of tasks to accomplish individual resident care.  The position is responsible to respond to emergency calls for the Adult Care Facility, observes and reports health and/or behavior changes to the RN and Physicians as needed, and supervises Home Health Aides and Personal Care Aides.
 
Salary range:  $26.00 - $30.00 per hour + $2.00 shift differential for evenings and overnights

 
 

Qualifications

  • Previous experience preferred
  • Must be currently licensed as a Licensed Practical Nurse in the State of New York.
  • Must be physically able to perform the essential functions of the job
  • Must be covid vaccinated

 

Responsibilities

  • Assists residents with their medications and performs prescribed treatments. Acts as a resource for questions pertaining to residents' medications.
  • Observes, reports, and documents changes in resident health and/or behavior.
  • Coordinates communication and documentation of resident conditions with physicians.
  • Assists in admission, transfer and discharge of all residents assuring proper orientation to the facility.
  • Evaluates and documents incident reports for residents.
  • Coordinates staff scheduling for coverage of Adult Care Facility if necessary.
  • Responds to emergencies within the Adult Care Facility.
  • Assesses employee incidents and need for further treatment. Properly documents incident reports by end of the shift. Report by phone incident details to Human Resources. Ensure accident reports have all signatures and are forwarded to facility Director.
  • Assists in orientation of new employees. Assures that new employees understand scheduling process.
  • Assist on the floor to provide personal care to Residents as needed.
  • Reports all changes in Resident status to RN.
  • Attends all mandatory in-services and other educational seminars as needed.
  • Acts according to procedures set forth for in the Fire and Disaster Plan.
  • Contacts Home Health Agency as needed.
  • Arranges lab work weekly and as needed.
  • Assists with insulin and B-12 injections.
  • Supervises staff routines in the Adult Care Facility. Counsels and/or issues counseling statements as indicated by the Director.
  • Evening and Night Shift - Functions as Facility Charge Nurse. Day Shift may function as Facility Charge Nurse if necessary.
  • Implements and tracks infection control systems.
  • Assists in proper communication of Quality Improvement philosophies and plans to all employees.
  • Understands and follows all HIPAA guidelines
