Salary range: $26.00 - $30.00 per hour + $2.00 shift differential | |
| At least one years’ experience working in the health care field. |
| Must be currently licensed as a Licensed Practical Nurse in the State of New York. |
| Assists residents with their medications and performs prescribed treatments. |
| Observes, reports and documents change in resident health and/or behavior. |
| Coordinates communication and documentation of resident conditions with physicians. |
| Offers suggestions establishing care plans for residents to maintain consistency and continuity of care. |
| Assists in admission, transfer and discharge of all residents assuring proper orientation to the facility. |
| Coordinates staff scheduling for coverage of Adult Care Facility if necessary. |
| Assists in orientation of new employees. Assures that new employees understand scheduling process. |
| Assist on floor to provide personal care to Residents as needed. |
| Supervises staff routines in the Adult Care Facility. Counsels and/or issues counseling statements as indicated by the Director. |
